Electro Surgical Procedures For Skin Diseases

Skin lesions such as warts, molluscum contagiosum, skin tags, milia, xanthelasma etc. can be easily treated with electro-surgical procedures. For this purpose, a variety of instruments are available, some of which have an arrangement for electrolysis also.

Basically, there are two types of instruments: (1) unipolar, and (2) bipolar, but both of these work with high intensity AC current. The tissue destruction is caused by the heat generated by the flow of the current through the tissues. The bipolar instruments have a neutral electrode usually made like a flap which is to be placed in contact with the bare skin of the patient. [Read more...]

Sipping Hot Tea Can Cause Throat Cancer

tea1People who drink their tea piping hot run a higher risk of throat cancer than counter parts who prefer a cooler cuppa, according to an investigation published on Friday by the British Medical Journal.

Cancer of the oesophagus is linked especially to smoking and alcohol abuse but hot beverages have also been considered a risk factor, possibly because of damage to throat tissue. Interested in finding out more, Iranian researchers went to Glestan province, which has one of the highest rates of oesophageal cancer in the world.

Inhabitants there sip large quantities of hot black tea typically drinking more than a litre per day per person but also have a low incidence of tobacco and alcohol use. [Read more...]

Epidermolysis Bullosa

epidermolysis-bullosa-genetically-inherited-diseases1Epidermolysis Bullosa is a group of genetically inherited diseases in which the patient shows an enhanced tendency to develop blisters on areas subjected to a minor trauma.

These diseases are also called mechano-bullous diseases.

The blisters are usually situated on pressure points such as the toes, fingers, ankles, knees, wrists and elbows and occasionally on the face as well. Areas of the body having a bone underlying the skin are the most frequently involved sites.

These blisters tend to heal rapidly unless they are subjected to repeated trauma or if they develop secondary infection. Some patients show ulceration of the mucous membranes and defective development of the nails. [Read more...]

Chemo Surgery or Chemical Cauterization of Skin Lesions

Chemosurgery means selective destruction of tissues using chemical agents. Chemsurgical procedures do not require any anaesthesia.it is also termed as chemical cauterization.

Such applications are usually needed

  • Chronic ulcers with unhealthy granulation
  • Flat warts
  • Resistant cases of chloasma
  • Papular lesions that do not respond to the conventional treatment.
  • Syringoma
  • Angiokeratoma

A lot of other lesions are also treated with this. [Read more...]

Surgical Incision Of Cysts And Abscesses

abscess-cystsAbscess due to pyogenic infections or cystic acne need to be incised to drain out the pus, because otherwise, this pus interferes with the healing process.

One of the method of doing this  by anaesthetizing the surface of the lesion with ethyl chloride spray.

This results in partial anaesthesia for a few minutes during which a sterilized knife should be inserted through the surface of the abscess at a point where the abscess is nearest the surface.

The line of the incision should always correspond to the imaginary lines of Langer to minimize the resultant scarring. For acne cysts, one should use a narrow blade knife usually employed in ophthalmic operations and make very small incisions so that it does not result in scars.

Following the incision, the lesion should be pressed on all sides to drain out the pus, opening up all the loculations if necessary.

A firm pressure on the lesion for a few minutes, as a rule, results in haemostasis after which an antiseptic dressing can be applied if the in haemostasis after which an antiseptic dressing can be applied if the lesion is big.

Depending upon the indications, the patient should be advised to take systemic antibacterial agents to promote proper healing.

Anti Bacterial Agents

Anti bacterials act either  by  killing the bacteria and are therefore, called bactericidal drugs or  inhibit their proliferation and are called bacteriostatic drugs.

tablets-antibiotics-drugs-medicineMost of these drugs act against a large variety of organisms and therefore, have a broad spectrum of activity, while others have a narrow spectrum.

Resistance is a problem with antibacterial agents and this happens more frequently with certain anti-bacterial agents than with others, more so, if the patient had been given subtherapeutic doses of the agent. Generally, an anti-bacterial agent should be able to control the symptoms within 2 days and eradicate the infection with 5-7 days, after which further treatment with the anti-bacteria agent can be stopped.

In case there is no improvement within 2 days, one should suspect resistance of the causative organism to the anti-bacterial agent. More appropriately, however, the causative organisms should be cultured in vitro to determine the sensitivity (susceptibility) pattern of the organism to various anti-bacterial drugs, and use the mot effective drug.

The anti-bacterial drug must be used in the prescribed dosage and continued till the infection has been completely eradicated. The exact duration of treatment will depend upon the type of infection, but it is preferable to treat a patient for a longer duration rather than stop the treatment too early. [Read more...]

Skipping Sleep Can Trigger Heart Disease

Just 1 hour extra shuteye reduce risk of calcium deposits in arteries, A precursor to an attack.

Just one extra hour of sleep a day appears to lower the risk of developing calcium deposits in the arteries, a precursor to heart disease, US researchers said.

The finding adds to a growing list of health consequences including weight gain, diabetes, high blood pressure linked to getting too little sleep. “We found that people who on average slept longer were at reduced risk of developing new coronary artery calcifications over five years,” said Diane Lauderdale of the University of Chicago Medical Center, whose study appears in the Journal of the American Medical Association. “It was surprisingly strong.” Calcium deposits in the coronary arteries are considered a precursor of future heart disease.

Unlike other studies looking at the risks of getting too little sleep, which use people’s own estimates of their sleep patterns, Lauderdale’s team set out to measure actual sleep patterns.

They fitted 495 people aged 35 to 47 with sophisticated wrist bands that tacked subtle body movements. Information from these recorders was fed into a computer program that was able to detect actual sleep patterns.

The team used special computed tomography, or CT, scans to assess the buildup of calcium inside heart arteries, performing one scan at the start of the study and one five years later. [Read more...]

Comedone Extraction With Comedone Extractor

black-heads-comedone-extractionComedone is another term for black heads. This simplest method available for removal of open comedones or black-heads is mechanical extraction by means of a comedone extractor. This is a metallic instrument which has a small spoon-like spatula at one end with central hole, while the other end is made into a handle.

The area of skin bearing the comedones is first treated with a pack of cotton soaked in two percent cetavlon solution for half an hour or so, so that the comedones become softened.

Following this, the comedone extractor is pressed on to the comedone taking care that the black keratinous plug is visible through the hole in the spoon-like spatula. This pressure leads to extrusion of the keratinous plug of the comedone and this has then to be repeated on every comedone.

If any comedone does not come out, it should be wetted again with cetavlon solution on the next sitting and subjected to the same procedure. In case the comedone extractor is not available, any metallic instrument with a ring at one end will suffice.

Pressing one edge of this ring on the skin and dragging it over the area bearing the comedones will extractor out many of the comedones.

Surgical Excision

surgical-excision-tumour-cysts-woundSmall tumours or cysts can be easily taken out by surgical excision. The area of skin bearing the lesion should first be thoroughly cleaned with spirit and then anaesthetized by injecting lidocaine under and around the lesion.

Once the area is anaesthetized, an elliptical incision should be given around the lesion, the long axis of which should correspond to the imaginary lines of Langer. [Read more...]

What Is Pulse Therapy?

doctor-and-patient-check-drug-schedule1Pulse therapy is given when long term steroids are to be given. It  consists of giving 100 mg of dexamethasone or 1.0 gm of methylprednisolone im 500 ml of 5%percent glucose by a slow intravenous drip in 1-2 hours on 3 consecutive days and repeating such pulses once a month till the desired effects are achieved.

This dosage schedule has been used for several diseases such as pemphigus and other bullous diseases, pyoderma gangrenosum, Reiter’s disease, systemic sclerosis, Darier’s disease, disseminated discoid lupus erythematosus, eruptive xanthomatosis, porokeratosis Mibelli etc. with good results.

The chief advantages of this regime include freedom from the common side effects of prolonged corticosteroid therapy such as obesity, osteoporosis, electrolyte imbalance, diabetes, hypertension, peptic ulceration etc, reduced hospital stay of the patient, and complete recovery from the disease in a majority of the treated patients.
